Prior to your first session, I will send you electronic documents via a client portal. This will will include informed consent forms and questionnaires for you to securely complete and e-sign online.
My practice management system is with SimplePractice, which has enhanced security features and is HIPAA compliant. The client portal will allow you to upload documents into your electronic health record, send secure messages to me, view statements, and self-schedule appointments.
Video sessions are conducted via Zoom Pro. I sometimes use Doxy.me or Google Meet as back-up options. All 3 platforms are HIPAA compliant and easy to use.
Prior to attending a sleep consultation, it can be helpful to keep a sleep diary for 1-2 weeks. This helps us to identify sleep patterns and habits. You may download this form for home use and bring the completed diary to your appointment or upload the file into your client portal.
It is ideal to complete the diary right as you wake up in the morning (for the previous night). Don't worry about being exact, just give your best guess regarding sleep or wakeful times, without watching the clock. In the notes section, you can record anything out of the ordinary as well as list any medication or sleep aids you used that night.

Please note, my suite is #222 and all the even suites are down one hallway and the odd suites down the other. If you prefer biking, there is easy access off of the American River Parkway bike trail. You'll find a bike stand in the parking lot between buildings 555 and 601.
