Please read "Welcome to my Practice" which explains informed consent for your counseling and information about privacy and confidentiality. Print and complete the "Initial Questionnaire" and the "Patient Health Questionnaire" and bring both to your first appointment.
If you have sleep concerns or insomnia, begin tracking your sleep in the sleep diary. Each morning, fill in the diary based on the previous night's sleep. You don't need to be exact (e.g. don't watch the clock), just give your best guess about the information.

Free parking on both sides of the building, with accessible parking in the front. Elevator is off the lobby. All the even suites are in one hallway and odds in another. Office is off the American River Parkway if you prefer biking. Exit the bike trail at the Sac State bridge and ride to 601 University or exit under the J Street Bridge and walk to the parking lot between buildings where you'll find a bike stand.
